The CARE Provider Network is honored to have been selected to provide you and your family a Preferred Provider Organization (PPO) option for your health care services. A PPO is a network of physicians, hospitals and other health care providers who have agreed to help control health care costs. Today, PPO's are one of the most popular health care delivery systems.

This is a guide to help you identify the medical providers that are in the CARE Provider Network. This directory of Preferred Providers is updated from time to time. There is no guarantee is that a particular provider will remain a Preferred Provider in the future or that he/she will still be a Preferred Provider when you seek services. There are also cases where a provider has joined the network but is not listed in your directory. We recommend that you contact the CARE Provider Network to confirm a provider's participation.
