· Read the booklet that you receive from your health care plan administrator for a detailed description of your health care plan benefits.

· Become familiar with any pre-admission certification or pre authorization requirements that may be included under your health care plan.

Do not assume that all services performed by a Preferred provider will be covered. It is your responsibility to verify that the services you are receiving are covered benefits under your health care plan.
